Team Physicians
Montefiore Medical Center |
The Athletic Training Staff works closely with a staff of orthopaedic physicians from University Orthopaedic Associates, which is housed within Montefiore Medical Center. Our team physician makes weekly visits to campus during the Fall and Spring semesters and as needed during the Winter months. If a student-athlete needs to see the doctor sooner than his weekly visit, arrangements will be made for an office visit. For more information about Montefiore Medical Center and University Orthopedic Associates, please visit: Montefiore Medical Center - Orthopaedic Surgery
Pre-Participation Medical Forms:
All student-athletes are required to have the following forms completed and submitted to the Sports Medicine Department PRIOR TO the first scheduled practice (in-season or out of season). Failure to do so will result in inability to participate with the team and receive treatment in the event of an athletic injury. Student-athletes can submit all of their forms through Sportsware.
